The DD (Developmental Disabilities) Wavier is a Medicaid funded program supporting individuals with developmental disabilities and acquired brain injuries. Case Managers Are Here To Help You Every Step Of The Way
Contact Rise Wyoming today to get started. Rise will work with you through the steps to complete the application with the Wyoming Department of Health. Here is what to expect in this stage:
1) Choose a Targeted Case Manager to help you through the next steps
2) Complete Medicaid Eligibility
3) Complete required psychological evaluation
4) Complete the ICAP (Inventory for Client and Agency Planning)
Once your application is submitted with all the required tasks, you will be placed on the waitlist until a funding opportunity is available. During this time, your Case Manager will connect you with local resources to fill the gaps until the waiver is active.
Once you are approved and funded , your case manager will oversee your Plan of Care through regular home visits and service check-ins. Your plan of care is a plan that is build by you, your team, and your Case Manager. This plan will address your overall well-being and goals that you have set for yourself.
